Governance needs memory

Migration programs make hundreds of small decisions: keep or retire, approve or block, map or redesign, automate or handle manually. Without a decision register, those choices disappear into scripts and meetings. When an issue appears after go-live, nobody can explain why a credential moved, why a product was approved, or why a portal user was archived.

What to capture

Record asset type, source identifier, target identifier, decision, approver, rationale, implementation status, and validation evidence. Include exceptions for app credentials, product associations, company mappings, and identity provider migration.
